You will be receiving a textbook as well. Sometimes the two work hand and hand so if that is the case, it will tell you in you study guide to wait for the textbook. Good luck!

See picture on where to get form to fill out and mail. Once you have clicked on Electronic forms it will be the next to the last link for the form. It has to be mailed not fax or email.
