Hm, I would definitely give PF a call to be absolutely sure your proctor form was approved by the school and that the correct mailing address, for your proctor, is on file. Verifying with the school will narrow down where the glitch lies - possibly, the packet with the proctored exams was lost in the mail. Best of luck!
